Volunteer Background Screening

Florida law requires every coach and volunteer to pass a Level 2 fingerprint-based background screening before working with players. This form collects the information the state needs to run that check.

Your information is encrypted in your browser before it is sent. It is stored encrypted, it is never visible to anyone but the league administrator, and it is deleted as soon as your screening has been submitted to the state.

What happens to the information on this form

Florida Statute 943.0438 requires this league to run a Level 2 fingerprint-based background screening on every coach and volunteer who has contact with players.

Your Social Security number is used for one thing: to identify you to the Florida Department of Law Enforcement so your fingerprints can be matched to the correct record. It is not used for credit checks, it is not shared with anyone outside the screening process, and it is never sold or used for any league purpose.

Your information is encrypted on your own device before it leaves it. It is stored as scrambled text rather than as something that can simply be opened and read, and access to it is limited to the league administrator's account.

It is deleted as soon as it has been submitted to the state portal. The administrator reads your record once, types it into the state screening system, and immediately deletes it. Anything not deleted by hand is automatically destroyed after 14 days, whether or not anyone remembers to do it.
